Moms are special, and taking the time to make something beautiful for them is well worth the effort. I have personally come to appreciate my own mother all the more now that I have my own children. I can see the many selfless ways she sacrificed for my sister and I and the many ways she continues to offer the gift of her love and time as we have gotten older. And I know my grandma did the same before her.

Instruction Guide

This free mother’s day questionnaire will come in a printable packet with 6 different name variations: Mom, Grandma, Nana, Granny, Grammy, and Mimi. Please note that this is not a physical item, but is instead a digital item that will be sent to you in pdf format. You will find the pdf information at the end of this post.

Once you have received the pdf file, all you will need to do is print it out on printer paper at home or via a local or online print shop and then ask your child each Mother’s Day interview question. They can either write down the answers themselves or you can write them out for them.

To finish, have your child draw their best picture of themselves with mom or grandma!

Terms of Use

While it is a free download, this printable questionnaire is for personal use only and is not to be used for commercial purposes. It would also be a great resource for classroom use as it is a fun mother’s day activity. Once you have your pdf file, be sure to save it so you can use it again for future years. This would make for a perfect annual gift as your child’s answers and creativity will change each year. Such a wonderful keepsake for mom or grandma!

Gift Ideas

These printable questionnaires pair perfectly with all kinds of gifts. You can simply print them out and add them to a cute card, or you could also get a little more creative. Here are some of my favorite ideas:

  • Print the questionnaire out on nice quality card stock and frame it
  • Add it to a fun photo board collage that mom or grandma can display at home or at work
  • Add it to a gift basket full of mom or grandma’s favorite things. You could also include some other homemade items such as a sugar scrub, bath bombs, or even baked goods!
  • Take a picture of your child’s completed questionnaire and print out the photo. You can frame it or add it to a little photo album full of sweet photos of your child and mom or grandma.
